Align VCL with meta repo

Done:
- Simplify logic around detecing hits as this seems to have been fixed
Varnish 4.0.3 and we req 4.1: https://github.com/FriendsOfSymfony/FOSHttpCache/commit/07cc5d7aec2bd02e464f34c42c8d70ffe2d27954
- Add more debug from meta repo, but make sure we check if obj.ttl is set before use
